This form is for January 2026 roster qualifiers only. All submissions will be reviewed and verified by WSB Staff in the order received once the form closes. If you have any questions, please contact the WSB Office.

  • If your numbers already show online and you qualify, you may submit as an early qualifier.

  • All verified qualifiers will receive an email from the WSB team with next steps by Thursday, January 15, 2026.

RECOGNITION

  • Please send a high-resolution photo of yourself (and your spouse, if you’d like them recognized).

  • The photo should include your full torso, and you should wear your MD jacket and an MD or GX shirt. Please avoid pin-stripe jackets.
